Basic capacities of US examination of the locomotion system in children

  • diagnostic of congenital dysplasia of hip joint, sub-dislocation and dislocation of hip joint in children of 1st year of life;
  • diagnostics of congenital disorders of locomotion system development (defects of bones and muscles, disorders of joint interrelation);
  • diagnostic of aseptic bone necrosis, osteochondropathies, functional transformation of bones;
  • diagnostics of arthritis, synovitis;
  • diagnostic of trauma injuries of soft tissues (muscles, tendons, ligaments) and bone damages (epiphyseolyses, osteoepiphyseolyses).

The basis of the method is the ultrasound examination, visualizing all components of hip joint: bone, cartilage, fibrosis-cartilage labrum, as well as allows defining interrelation in a joint and to perform functional probes in real time mode, assess stability thereof. The method of hip joint US examination in newborns and children under 1 year old is elaborated in details and described in many publications in different languages by this method’s founder R. Graf (Austria).

US examination of hip joints in our department was firstly applied in the Soviet period, and we have the longest experience in this sphere.

In Ukraine the regulation on hip joint US as a part of Protocol on diagnostics and treatment of hip joint dysplasia has been approved by the Order of Healthcare Ministry of Ukraine on 26.07.2006 No. 521. Although in our country the decision of the method of choice in mostly made by parents. The most efficient is diagnostics at the age from 1 to 4 months; this is stipulated by possibility of early and efficient correction of disorders in case of finding thereof.

Early diagnostics allows avoiding hip joint arthritis, disability and ensures full extent of life.

The R. Graf’s method itself is well-grounded and leaves little space for free interpretation, although many issues depend on accuracy of measuring and following the protocol of examination. The present discrepancies in statistics by different doctors are explained by violation of the protocol, and it depends on the experience of expert.

To define the importance and expediency of the study, one should consider the following risk factors:

  • breech presentation of the baby while pregnancy;
  • prematurity;
  • Reasonable weight of a child – over 4 000 g;
  • Existence of the factors of inheritance relating to locomotion system’s diseases.
